In consideration of my interest in furthering Wisdom Healing Qigong, I hereby release rights to my image, likeness, and sound of my voice, as recorded on video, audio, and/or photographs during my enrollment in The Chi Center Passages program. 

I understand that all digital recordings, videos, and photographs will be exclusively viewed by members of the current Passages program, as well as used for internal Chi Center training purposes.

I hereby release the Chi Center, and any of its associates or affiliated businesses or agents, such as their directors, officers, and employees from any and all claims of every kind on account of such use.
